Clinicians
LIST OF REQUIREMENTS
- Copy of Driver’s License
- If not available, copy of passport or permanent resident card
- Social Security Card Number
- Copy of Current Professional License/Certification
- Copy of Current CPR Certification
- Certification must be through the American Heart Association, or at least completed under the guidelines of American Hear association
- Copy of up-to-date automobile insurance card
- Copy of current physical examination
- Must be undergone within six (6) months from date of hiring in order to be considered “current”
- Copy of Annual PPD Test
- If positive result:
- Submit Copy of PPD test with positive result
- Submit Copy of x-ray with negative result
- If positive result:
- Copy of current professional liability insurance (applicable to independent contractors only)
