Step 1: Fill out the application form.

Step 2: Register as a licensed caregiver on this webiste: www.ccld.ca.gov. Then download the form for Home Care Aide Registration.

Step 3: Download this form (Live Scan Service) This is a request for live scan service. Fill out the form, and bring the form to UPS and they will do the live scan. UPS won't process the live scan if you don't bring the form with you.

Step 4: Download this form (Employees Statement) If the office has already contacted you for a scheduled interview, please download and fill out this form and bring it with you on the day of your interview.
